Our philosophy is to provide high quality and accessible health care through partnership; by matching resources to patient needs; and by caring for staff through structured support, education and training.

We particularly target our efforts at areas of unmet need.

We believe that high quality primary healthcare depends upon three key principles which underpin our model of service provision:

Accessibility

All patients are able to access high quality primary health care, irrespective of age, sex, ethnic background, language or culture. Services therefore are tailored to the local need and are developed by the local community in partnership with ourselves and other health professionals.

Excellence

Many excuses are made for what is often poor quality primary care. We believe that the care that is provided, no matter what the area and local health needs, should be of the highest quality.

Partnership

Modern primary health care involves delivery of complex and diverse services. Good primary care can only be provided by strong, supported and diverse team working in partnership with patients; professionals; Commissioner and other stakeholders. For Inclusive Health, this means that as much effort goes into managing the processes of health care provision as goes into the outcomes.

Enhancing Patient Access into General Practice – The NWL Model

Across North West London, General Practices & Primary Care Networks were asked to develop an access model that enhances patients’ access to GP surgeries. The aims of the specification that North West London developed are to make effective use of resources, increase use of digital tools & increase proactive care for those that need it.

As part of the specification, we submitted a delivery plan, which includes:

  • Conducting a monthly audit of our appointments to ensure their appropriate use.
  • We aim to answer all incoming telephone calls within 10 minutes & want to reduce the overall waiting time for our patients.
  • Improve continuity of care between patients & clinicians by ensuring patients are seeing their preferred GP for ongoing health concerns.
  • Increase of use & efficiency of our online consultation platform, PATCHS. Our practice teams have undergone Care Navigation Training to be able to help patients with their requests & to book their appointment with the most appropriate clinician.
  • Engage with our patient population by means of a PCN-wide Patient Participation Group (PPG). This will enable us to communicate better what is happening within the PCN as well as what services are available for our patients to use.
